為確保療程順利，請先備妥以下證明文件：
已於台灣戶政機構完成登記：
請提供「身分證」或「結婚證明書」。
於大陸/港澳結婚：
須提供經行政院設立或指定之機構（如海基會、我駐港澳辦事處）驗證之結婚證明文件。
於國外辦理結婚或外籍夫妻：
須提供經我國駐外館處驗證之結婚證明文件。
溫馨提醒
1. 若無法提供合法證明，醫療機構依法不得施行人工生殖技術。
2. 須於台灣戶政機關完成結婚登記，才可申請「試管嬰兒補助」。

According to the Taiwan’s Artificial Reproduction Act, assisted reproductive treatment is limited to legally married couples.
To ensure a smooth treatment process, please prepare the following documents in advance:
If your marriage is registered with a household registration office in Taiwan:
Please provide your National ID card or Marriage Certificate.
If you were married in Mainland China / Hong Kong / Macau:
You must provide a marriage certificate verified by an institution established or
designated by the Executive Yuan (e.g., Straits Exchange Foundation, SEF or Taiwan’s representative offices).
If you were married overseas or are a foreign couple:
You must provide a marriage certificate authenticated by an ROC overseas mission.
Friendly Reminders
1. If valid legal documentation cannot be provided, medical institutions are not
permitted by law to perform assisted reproductive procedures.
2. To apply for the IVF subsidy in Taiwan, your marriage must be registered with a
Taiwanese household registration office.
